4. date required for shipping and/or pickup

6. what type of garments are we printing? (Tees, Hoodies, etc.)

7. What color garments are we printing? (Blk, wht, etc.)

8. How many different prints? (Front, chest, back, etc.)

9. how will you be supplying the art?
